Transcript Request Form

All persons who desire to attend PHSBC must provide an official transcript from all post-secondary education as well as roof of high graduation or GED.  Please complete the Transcript Request Form (below) to request an Official transcript from your high school and college(s) to be mailed to PHSBC.

Additional Documents Required:

• A brief written testimony of your salvation experience
• A letter of recommendation from your pastor or ministry leader
